Welcome to Stewartry Self Management Programme

Self Management Programme (SMP) has been established in five different areas in Dumfries and Galloway in 2012.
The programme is part of Building Healthy Communities and is supported through the Change Fund Programme ‘Putting You First’.
It focuses on individuals approaching older age 50+, those living with long term conditions and carers. Self-Management is a term used to describe better management of circumstances or health issues by individuals themselves.

For more information on Building Healthy Communities and Self Management Programme in other areas, please click on the links lower down. Thank you.

Healthy Connections Stewartry

The project links those 18 years and over attending GP consultations, in particular, individuals with long-term conditions and those with mild to moderate depression and anxiety with wider supports and services within the local community to improve health outcomes. Some examples of activities available are art activities, low level physical activity, exercise on referral, employability and financial advice, volunteeering opportunities, self management courses, lifelong learning, healthy reading catalogue and the listening service.

If you would like to know more about the project and how it can support you please contact Annemarie Sim, Self Management Programme Worker or Sharon Walker, Public Health Practitioner on 01556 505724. You can also speak to your GP or Practice Nurse if you are a patient of Dalbeattie Clinic or Castle Douglas Medical Group.
